Please fill out this form if you have any individual wishing to attend the West Tennessee Awards Program, but does not wish to purchase a meal ticket. These ticket recipients will be able to view the awards program from the balcony seating.  Chapters will be billed by the West Tennessee Awards Program for the total number of tickets needed at $5.00 each.
