YOU NEED THE MEMBER IDENTIFICATION NUMBERS FOR EACH STUDENT IN ORDER TO COMPLETE THIS FORM.
YOU WILL HAVE TO ENTER EACH INDIVIDUAL MEMBER ON A SEPARATE FORM.

Please enter the following information that must be submitted to the State Department of Education. Be sure to spell names correctly as this is how the names will appear on the member's plaque.

Please register all candidates by February 5th, 2024 before 12:00am. You must also bring one copy of each application to Crockett County High School on February 6th, 2024 or the candidate will not qualify to receive their State FFA Degree.
